Social Services
Social Services are offered at the Pender Community Hospital through the Discharge Planner. This service is available to all patients at PCH. The Discharge Planner works closely with the physician to meet the needs of our patients. Patients may need assistance in obtaining durable medical equipment, home health care, private caregiver, placement in nursing home or assisted living center, pastoral services, and help in applying for financial assistance. Other services may be needed and the Discharge Planner will assist the patient in finding the necessary resource to meet their needs.
