Pender Community Hospital's new Geriatric Assessment Clinic opened in late May. The mission of the clinic, which is based in the hospital, is to "help the elderly in our communities maintain and optimize wellness and independence through a multi-disciplinary approach."
The Pender Geriatric Assessment Team, headed by Dr. Matt Timm, will perform an assessment on patients referred to the clinic by their physician. The assessment will identify health concerns and arrange for services elderly patients need to continue living independently. Patients, their families and their family physician will receive the team's complete report of its findings and a personalized care plan.
Dr. Jane Potter, Chief of Geriatric and Gerontology at the University of Nebraska Medical Center and her geriatric team worked with the Pender Geriatric Assessment Team to organize the clinic. For more information about the hospital's newest clinic, call (402) 385-3083.
